This is a new thing. On Xbox you now have to press X to view it, and on Switch press Y. Try pressing [ ] on PS4 while hovering over the item (only in survival mode too)
You have to build the new portal in the Nether. I think this has become one of those “it’s not a bug it’s a feature” things.
