These game breaking bugs are not acceptable, should have been fixed when 1.14.2 came out.
Confirmed on 1.14.2. Tried switching between peaceful and hard, nothing. They stack you forever anywhere you go

It was just ground, no lava or fire.

I did die, the explotion killed me. Both times I went to the place, looked in the zone activating the hitboxes and didn't see the tools, they were not there.
Pickaxe: Efficency IV, fortune II, Unbreaking II

Drivers updated, problem solved. Thank you. It was only happening on this minecraft versions tho.

The other versions work completely fine for me. I have experienced this issue only with the 1.13 snapshots and pre-releases.

i have the same issue with 1.8 and 1.9 and the Mojang Support Center is completely useless, there's nothing related with this issue there
