steps to reproduce

1. go to the end

2. break the 1 layer of endstone under exit portal, leave no endstone

3. go away around 100 blocks wait a minute

4. reload the world and go close to it

5. if island doesnt regenerate repeat steps 3,4,5

this effects 1.20.72 im on windows 11 but im sure this works on all platforms of bedrock
