same issue, it not working with Passwort (yes correct even wrote it down),

however, after using the 'send me a code' variant, I was able to log in once more.

same Idwould generate if the water wasn´t here probably one block thick

I searched until I saw the airblocks in the water ( placed a scafholding to show the water hight) 

default world settings creative mode

found one spawning in a tree that sufocated  just after I first time loaded the Chunk

(Changed the siye of the image)
