The hearts wave normally like that, same goes with regeneration potions.

Also the piston glitched is on a timer.
UPDATE: I found out the vertical sticky piston acts like a solid block, even though there is not 2 pistons. I also broke the non bugged piston, and the vertical part of the piston was still there.

It could be a texture glitch, try creating another world just like that one with default textures and stay on same version, does it still happen?
