Started a new world and even if I have the item in my inventory, the item goes to a different spot like it’s not the same item. Even if the stack isn’t full, it will start a new stack, no matter the item.

Only modifications I have are the vanilla tweaks but it happens when I turn them off too.
It is not a LAN world or multiplayer, it’s single player.
Between 1.21.10 and 1.21.11, food items gained additional item components, increasing their total component count. (i.e. steak from 12 to 14)
I wasn’t able to reproduce this issue, as updating directly from 1.21.10 to 1.21.11 changes the component value from 12 to 14.
This leads me to believe that the behavior may be caused by updating from a snapshot between 1.21.10 and 1.21.11, where the component value is specifically 13, to 1.21.11.
Side note: If I am correct, you should not have an issue with the food stacking once you get rid of all the component value 13 food.
It is will all items, not just the food. They refuse to stack in existing slots that contain the item.

However, this issue is Invalid.
Your game, launcher or server is modified.
If you can reproduce the issue in a vanilla environment, please recreate the issue.
Any non-standard client/server/launcher build needs to be taken up with the appropriate team, not Mojang.
A world which had previously been opened with a modified client might be corrupted. Please try reproducing the issue in a newly generated world.
Any plugin issues need to be addressed to the creator of the plugin or resource pack.
If you have problems on large servers like Hypixel, please contact them first as they run modified server software.
